for the Member Exhibit. Forms must be submitted no later than May 1st,
2023, to be included in the MVC 2023 Member Exhibit.
Please indicate the total number of entries you intend to submit, between 1 and 5. Note that you must submit a separate entry form for each, and pay for your entries through the Conference Registration site.

I can submit up to one (1) entry in each of the the five (5) categories.
Entry Fee is $15 for 1-2 items, $25 for 3-5 items; no refunds will be issued if items are not submitted. Fees are payable through the Conference Registration site.
Submission of an entry constitutes my permission for Midwest Weavers
Association to photograph the work for publicity purposes.
In order to be included in the judging for awards:
(a) I must deliver my work to the conference committee no later than 9:00am on Thursday, June 15th;
(b) My work must remain on display until 3:00pm on Friday, June 16th; and
(c) I, or a designated proxy, must personally pick up my work before the end of the conference, OR
(d) I must supply the committee with self-addressed, prepaid shipping materials to send the entry back to me.
Each artist is responsible for insurance coverage of his or her work.
I understand that exhibits will be carefully monitored by MWC volunteers.
