Instructions

Forms must be submitted no later than May 1st, 2023, to be included in the MVC 2023 Guild/Study Group Exhibit.

Please provide the name of the guild or group submitting the entry.
Please indicate the town/county and state in which you group is located.
Please provide the name of the person responsible for this entry.
Address(Required)
Please indicate how many 6-foot tables your exhibit will require.
Please enter a number from 0 to 2.
Please indicate when you expect to deliver your entry to the exhibit organizers.
Please indicate when you expect to retrieve your entry from the exhibit area.
Please provide the name and mobile phone number of the person who will retrieve the entry at the end of the exhibit, if someone other than the Contact Person/Submitter.

As the above-named Contact Person for my Guild/Study Group, I understand and agree that:

  • I can submit up to one Guild/Study Group entry.
  • Entry fee is $15, Please make a check payable to “MWA Conference 2023” and send to:

    Please indicate 'Guild Exhibit Entry' on the check.
  • Submission of an entry constitutes my permission for Midwest Weavers Association to photograph the work for publicity purposes.
  • The Conference Committee will provide up to two 6-foot tables for exhibit use.
  • Each Guild will be responsible for setting up their own Guild display.
  • I, or a designated proxy, must personally oversee the check-out and removal of the exhibit by the end of the conference.
  • Each Guild/group and/or individual artist is responsible for insurance coverage for items included in the exhibit.
  • Exhibits will be carefully monitored by MWC volunteers.
